How do I access the electronic health record (EHR)?

After submitting your information to schedule an appointment on our website, an electronic chart is automatically created in our system. An appointment is made and a link is then sent to your device to register on the patient portal. Use the cellphone number you originally submitted as your confirmation/security push code, and you will access your chart and complete the short registration document describing your medical condition. When that is completed by your, the provider will be alerted and will send you a link for your teleconsult.

How does your service work?

We offer a consultation service. As registered and certified providers with the NYS Office of Cannabis management, our health care providers will speak with you about the condition that you have already been diagnosed with and make the recommendation on your behalf to become certified to use medical marijuana to assist in the treatment of your health condition.

How much does it cost?

The cost for the tele consult and registration with the NYS Office of Cannabis Management is $199.00. This includes the certification document (PDF) that you can present to the dispensary when purchasing the products you need. There are added costs for express service or other letters client may request related to the certification.

Do I need to produce any documents for this visit?

Our providers do not diagnose clients with a medical condition. Clients have already been diagnosed. Presenting any evidence of a diagnosis or treatment plan will support your application for registration. You will also need to show ID proving that you are a New York State resident.
